2019-2020 Fall Registration

Please use the form below to register your student for classes. You will have the ability to register up to four students at a time. Once completed, you will be allowed to proceed to the next step for payment of the corresponding registration fee. Registration is not complete until the registration payment has been made. All registration payments are nonrefundable. Please repeat the forgoing steps as necessary to register additional students. Registration is based on a first come, first served basis, and classes will close once capacity is reached. Please note, the registration fee of $35 per student will increase to $40 per student after classes start on Sept 9, 2019. Open Registration will close and no students will be accepted after January 1, 2020 due to the beginning of recital rehearsals.  Thank you for choosing the Walker School of Ballet!

Payments Due 2019-2020 *
*Registration Fee: $35.00 | $40.00 after September 9th, 2019 *Monthly Tuition is due on the 1st of each MONTH, and after the 10th a $10 late fee will be applied to your account. *Additional Fees are required prior to participation in recital performances. All students are encouraged to perform in the recital at the end of the season, but it is not mandatory.
Tuition *
Tuition is Non-Refundable and payable on the first of each month. Monthly Tuition will be invoiced to the email provided at the time of registration unless otherwise directed. Any over payments will be applied towards future tuition and will be considered Non-Refundable. Accounts that are not brought current by the 10th of each month will be considered late, and will be charged a $10.00 per month late fee. If the account is not brought current by the following month, the student will be asked to sit out and not participate in class. All checks returned for insufficient funds will be charged a fee of $35.00 and will be billed to your account, with cash or card being the only form of payment accepted thereafter. Notification of withdrawal of your student from classes must be in writing and given to the director of the school. This dance season contract is valid from September 9, 2019 thru June 9, 2020. We will be following the Jackson County School Systems calendar for all major holidays on which classes will not be held. No refunds will be made for unattended classes. Make-Up classes will be offered if the student has been ill or experienced a family emergency. Make-Up classes must be scheduled and approved through the director of the school and made-up by May 31, 2020.

As legal parent or guardian, I release and hold harmless Walker School of Ballet and its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage,or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Walker School of Ballet and its owners and operators or in route to or from any said premises.
Medical Emergency *
The undersigned gives permission to Walker School of Ballet and its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental disabilities, restrictions, or conditions and/or declared the participant to be in good physical and mental health.
Social Media- Picture Permission *
I give Walker School of Ballet permission to use my child(s) picture for the purpose of advertising on the studio website, social media and/or informational brochures or posters. If for any reason I do not feel comfortable with this, a written notice to the school will ensure my child's picture will not be used for any reason.
